Thank you so much for taking the time to complete this Recital Registration Form. We are so grateful that Dual Star is able to put on a recital every year and we hope your dancers enjoy the experience as much as we do!

The first registration will begin Monday, November 18th and end Friday, December 20th. If you register after 12/20, you will be charged an additional $20 shipping and handling fee and risk not receiving the size requested. We cannot guarantee your costume will arrive in time if you order after April 1st.

As we get more information and details we will send out emails as well as update our Recital Page on our website.

Please measure your child’s girth, bust, waist, hips, and inseam with a standard measuring tape. In order to take the most accurate measurements possible, click to watch this measuring tutorial.

We use 2 different costume vendors. Please review each chart below and determine what size you want to get your dancer from Chart R and Chart W *XLC and XXLC sizes are made to fit a larger child, not a petite adult

COST BREAKDOWN
If you are doing 1-3 dances, the set fee is: $120.00 per costume. Cost includes $100/Costume + Recital fee which includes a prorated June Tuition of $20) Sales tax included in price *Excludes any company dances

Set fee if doing 4, 5, 6 or 7 dances: $110.00 per dance. Cost includes $100/Costume + Recital fee which includes a prorated June Tuition of $10).

For example if dancer is dancing in 4 recital dances: Costume 1 = $120, Costume 2 = $120, Costume 3 = $120, Costume 4 = $110. Total is $470.00 Sales tax included in price *Excludes any company dances

Set fee if doing 8, 9, 10, 11 or 12 dances: $100.00 per dance. Cost includes $100/Costume + Recital fee which includes a prorated June Tuition of $0) Sales tax included in price *Excludes any company dances

Terms and Conditions*
Dual Star Academy of Dance is NOT responsible for each child’s measurements and costume size selection and does not take responsibility for costumes that do not fit. We have been warned of possible shipping delays. If you register after December 20th, you will be charged an additional $20 shipping and handling fee and we cannot guarantee you will get your costume on time. Each costume that is purchased is the child’s property and is theirs to keep. We do not require you to return your costumes post-recital. Dual Star Academy of Dance will not provide any refunds on costumes ordered unless a mistake has been made on the administrative end. Children/Account holders who originally register for a recital number, but drop out of the class pre-recital will not receive a refund for any costumes ordered. Fees will be charged at the time registration is received. Any costume payments made by cash or check will require those funds to be turned in to the front desk prior to the registration deadline. If payment is not received within the allotted time, Dual Star will discard the recital registration form. If a credit card payment is declined, account holders will have to update the credit card information on file via mydanceworks.net or by contacting the front desk. If a credit card is NOT updated prior to registration deadline, Dual Star will discard the recital registration form. Dual Star Academy of Dance is not responsible for any lost or stolen items during the dress rehearsal and recital performances. Any belongings left at the venue and collected by Dual Star Academy of Dance staff will be held at the studio for ten (10) days post recital. Any items not claimed within the allotted time will be donated to charity.
